Athletic physicals will not be offered at Moody Middle. If you plan on getting the physical at your pediatrician, please make an appointment as soon as possible and take a copy of the VHSL physical form with you. See below for a VHSL link to get the correct physical form. No other form will be accepted. Your child will not be allowed to try out if they do not have a completed physical on the first day of tryouts. The entire form must be completed and signed. If items are left blank, your child will not be able to try-out. Insurance information is required to be on the physical form. Please make a copy for your records.

**New VHSL Policy Henrico’s MSAC (Middle School Athletic Council) will follow this new policy effective July 1, 2025.

The Virginia High School League (VHSL) changed its policy for athletic physicals effective July 1, 2025, to a 14-month validity period. A student's physical will now be considered valid for 14 months from the date the healthcare provider's signature appears on page 3 of the VHSL Athletic Participation/Parent Consent/Physical Examination Form.
